Premium Clean New Zealand's Free Cleaning Services for Cancer Patients - Terms and Conditions

Effective Date: May 28, 2024

In consideration of the opportunity to participate in Premium Clean New Zealand's free house cleaning program for cancer patients, I, and my heirs, successors, and assigns, at this moment, agree and represent as follows:

Voluntary Participation

I understand that residential cleaning partners participate in this program of their own free will and are not obligated to provide services, nor do they get paid or reimbursed for their services. Participating cleaning services agree to perform a general clean only and pay their employees to perform this service free of charge to me.

Eligibility and Legal Competence

I am 18 or older and legally competent to enter this binding legal contract. I understand that participation in Premium Clean New Zealand's free house cleaning program is strictly voluntary, and I freely choose to participate.

No Warranty

I understand that Premium Clean New Zealand makes no warranty, express or implied, regarding the quality of the cleaning services provided by its cleaning service partners under this program.

Service Provider Representation

I understand that Premium Clean New Zealand employs and compensates the professional cleaners in my home, ensuring that they are trained professionals.

Release of Liability

I agree to release and covenant not to sue Premium Clean New Zealand, its officers, employees, agents, successors and assigns from any liability, claims, demands, losses, or damages on my account that are caused or alleged to be caused in whole or in part by the negligence, directly or vicariously, of Premium Clean New Zealand, its officers, employees, agents, successors, and assigns.

Insurance Coverage

I understand that Premium Clean New Zealand does not provide me with any insurance coverage, including property, liability, or medical. Although the cleaning service partners represent themselves to be insured and bonded, Premium Clean New Zealand does not attempt to verify or ensure this. I understand requesting proof if I want this assurance.

Non-Recruitment Agreement

I agree that I will not hire or attempt to recruit employees of a cleaning service partner to perform services for me independent of the cleaning service partner.

Jurisdiction and Governing Law

I understand that the laws of New Zealand shall govern any interpretation of this release without regard to the choice of law conventions of the forum state.

Confidentiality

All information provided by applicants will be kept confidential. We are committed to protecting your privacy and will not share your information with third parties without your explicit consent.

Service Availability

Applications are subject to approval based on the availability of our service in the area/location. We only service Auckland, Christchurch, Dunedin, Hamilton, Hastings, Napier, Nelson, Palmerston North, Rotorua, Tauranga, and Wellington.

Service Limitation

We can only cover 2 hours of free cleaning service. Regular cleaning rates will apply if you need our cleaners to clean for more than the provided time. This cleaning service is for regular house cleaning only.

Eligibility Criteria

To qualify for Premium Clean New Zealand's free cleaning services for cancer patients, applicants must meet the following criteria:

• Applicants must be undergoing active cancer treatment.
• Applicants must reside in one of the following service areas: Auckland, Christchurch, Dunedin, Hamilton, Hastings, Napier, Nelson, Palmerston North, Rotorua, Tauranga, and Wellington.

Application Process

• Application Form: All applicants are required to fill out the application form provided on our website. This form includes the necessary information to verify your identity and medical case.
• Supporting Documents: Applicants must submit proof of ongoing cancer treatment, such as a letter from a healthcare provider or a treatment schedule.
• Approval Process: Applications are subject to approval based on the availability of our services in the applicant's location. We strive to accommodate as many eligible patients as possible, but acceptance is not guaranteed.

Service Details

• Service Scope: This offer includes up to 2 hours of free regular house cleaning services. If additional cleaning time is required, it will be billed at our standard cleaning rates.
• Service Type: The cleaning service provided is for regular house cleaning only. Specialised or deep cleaning services are not included in this offer.

Confidentiality and Privacy

• Confidentiality: We will keep all personal and medical information provided by applicants confidential. We are committed to protecting your privacy and will not share your information with third parties without your explicit consent.

Service Limitations

• Geographical Limitation: Our free cleaning services are currently available only in the specified locations. We are continuously working to expand our service areas to help more patients in need.
• Availability: The availability of our cleaning services is subject to our current capacity and scheduling constraints. We will do our best to accommodate your preferred times but cannot guarantee specific appointment slots.

General Conditions

• Non-Transferability: The free cleaning service is non-transferable and can only be used by the approved applicant.

Compliance with Terms: By applying for and using this service, you agree to comply with these terms of service. Failure to adhere to these terms may result in the termination of services.

I have completely read and understood this Patient Release of Liability. I am submitting this release and waiver of liability voluntarily and of my free will. I agree to be bound by the terms of it.
